no application / enrollment or eligibility is required. Simply print the
FREE card below and receive savings of up to 75 percent (savings average around 30 percent) at more than 54,000 national and regional pharmacies. Participating pharmacies
(which are subject to change) include CVS/pharmacy, KMart, Target, Walgreens, Winn Dixie, WalMart and more.This card can be used as your primary plan and / or be used for prescriptions not covered by your insurance plan. The discount program also includes other value-added programs for eyeglasses, hearing aids, nutritional supplements,diabetic supplies and more (see below). You may print as many cards as you need - feel free to print cards for family and friends. The card is pre-activatedand can be used immediately.
